Why can't it be both? You think a rational, mentally healthy woman would kill her kids because she was jealous/mad at their father? Mentally ill people can absolutely still have motives and thought processes. Those thought processes are going to be screwed up and irrational, though.

This isn’t just about depression. Sinead, for instance, thought she had bipolar disorder, and now believes she’s dealing with PTSD instead. There’s also borderline personality disorder. Coping with BPD or bipolar is not the same game as clinical depression
