Your HBO examples are pretty weak. Ned was GRRM like you said, there's a lot of speculation that Jimmy was written off because the actor was a jackass, and Nate lived until three episodes before the end of the series.
I don't know how you didn't pick up on the fact that Carrie did that on purpose. She even spelled it out in the conversation with Quinn afterwards.
