I think you over simplify it a bit. I think a lot of people like the rewards systems but draw the line at playing games they don't like to get more or playing bad games to death to get the game 'complete.'

Well most orange loot drops from a specific boss. These bosses probably have an orange item too - it's just they don't drop every time. All most every named boss type drops a specific orange item. You may have to kill these a few times to get what you want.
This one really needs to raise the cap - I think most players probably have at least one character at level 50.
Ads are verywhere - cable tv you pay for and still get ads. I think it is a similar model. Not saying I agree with it. But we pay for movies, cable, sports events and all have ads as part of the package.
