Miller actually started in comics as an artist, and he drew a lot of his own stuff - he drew all of Sin City, and The Dark Knight Returns. Unfortunately, he also drew The Dark Knight Strikes Again, which is one of the ugliest comics I’ve personally wasted time on.

Narratively it's padded armor to help with the anonymity of whoever happens to be Ronin at the time but the reality is bad comic book art.

That has long been a Frank Miller trope. I dunno if that particular panel is Miller (it doesn’t look ugly enough), but you can see that thing all through his Daredevil, various Batmans (Batmen?), Wolverine, etc.

Nah. Clothing is lighter, less inert, more flexible than flesh. Tenting like that would make perfect sense. (I am not, myself, a trained assassin or serial killer, I promise.)
