I really don’t think saving Ellie is supposed to be a “fucked up terrible thing”. In fact that’s one of the only really good things he did in the whole game. It’s his redemption.
All we can say for sure is that if the fireflies were being honest Ellie would’ve sacrificed herself and left Joel behind while Joel wouldn’t be able to carry on if that happened and wouldn’t let Ellie go through with it.
Developers have already confirmed that Ellie’s death would have saved everyone from the plague. People try to downplay that aspect so that the selfish answer is the only correct answer. But it is not. Thousands of “Ellies” will die because of what Joel did. Humanity will suffer a lot.
