Yeah, the doctor in the ER are super busy, so you are asked these questions before they even see you. That way they have all the info in front of them and don't waste time taking a survey. He should have known that she had no idea she was pregnant.

I think Season 1 Hannah would have had an abortion just for the kick of it, for the experience, to have something to write about. Season 6 Hannah will have an abortion because it's the right, mature thing to do.
