I'm not sure that the Endless are actually part of the DCU Creation. I think they do transcend it as you suggest. They are ideas more than anything else.

Does the Mike Carey spin-off Lucifer count as separate from the Sandman one? I know the continuity is the same, but Lucifer feels a lot different in Sandman than in his own title.

Yeah, the theology in Sandman is pretty fluid (probably part of the point). However, the God that appears in the spin-off Lucifer is presented as the definitive creator of the universe.
