They're what you get when you don't know how to drive but need proof of age and/or identity (airline travel, cigarettes, what have you) and don't have or don't want to carry a passport. Not that I'm speaking from personal experience or anything.

Also, in some of those states the doctors aren't actually state residents, but doctors who travel around to various clinics - so hospitals will deny them admitting privileges because of that.
