It was fixed in FO 4 by a player made patch, or a mod as one might call it. Thus Bethesda never fixed it themselves, thus it had to be fixed in FO76.

Hacking can be easily dealt with like it was in D2 by having a single player mode for those who want it and online for those who want multiplayer and/or no hacks.
