I've seen spectres of people dying in combat, in the dream. I wasn't sure if maybe they were attacking the Doll or Professor X or whatever. I recall being able to attack and kill NPCs in the other Souls games.

Because the Butcher has been officially announced as a character they are working on for the game. No one knows how his kit will be different, because like you said the hook is the same. But they are working on him.
