Airlines and flying is all about the weight. Cut out the weight, to save fuel, reduce costs. If it isn’t necessary, it’s gone. So maybe “back in the day” it would have been a thing. or on a luxury carrier, but never on a “regular” carrier in 2015.

There are only a limited number of hot meals loaded onto the airplane. If they were all given out to the passengers in first before the family made the request, I can see it not being available. And contrary to popular belief, there are not microwaves onboard to heat the food.
