In some ways, doesn't it seem Cooper's Vegas world that surrounds him is acting even more surreal than HE is? i.e. no-one actually acknowledging he should be in a facility of some sort. Perhaps, as someone else here mentioned earlier, none of it is "real," or at least the reality we are waiting for him to return to.
