According to IOC rules, a T/E ratio over X (some sources say 4.0, some say 6.0) is an offense unless a definite medical condition can be given for excessive T production. If you are above the ratio and you do not have medical justification, you are breaking the rules whether or not you are actually taking anything

FYI, because you piqued my curiosity I decided to read through some WADA regulations regarding the T/E test, and they do make special exceptions for those with genetic conditions predisposing them to failed T/E tests.

The answer to your question is that any person with abnormally high levels of testosterone will be suspended. They don't distinguish between natural and unnatural.

Again, I am not 100% versed on the rules, but that is my understanding of how drug testing works.

T-levels are one of the things any drug test for athletes will examine. Whether or not they end up finding more evidence beyond the T-levels, I am not completely sure, but anything over 6x (may vary between organizations) the average male T-levels will trigger scrutiny. As far as I know, they can’t tell the difference
