There were walkers on the other side of the fence. That's why they didn't scale it and why Nicholas saw no way out and shot himself.
Glenn expected Nicholas to know the layout of the town and explicitly stated his expectation that Nicholas would guide them out of there safely. Why?
When herding cattle or sheep, the shepherd often has a sheep dog to keep the strays in line. Daryl's the dog in this scenario.

What do you mean by, "the geography had been altered"? The feed store was burned, but its remains were in the expected place. I don't recall any other changes that would disorient him.
Rick wasn't brutal. He was defending himself.
Glenn relied on Nicholas to direct him to the best route toward safety. Nicholas choked, and (seemingly) randomly picked the direction they took. Which, like the unsafe alley Nicholas had led the larger group down earlier, was a poor call.
And to their chompers, which work surprisingly well, given how fragile their skulls seem to become.
