There are a few ways: a late script or code change introduced a bug (maybe even in a change in Havok). Perhaps it was noticed too late to make a change. Maybe it was placed on a backlog and more important issues were addressed and the release date came before getting to it. Maybe it wasn’t considered a game-breaking…

Not only that, but the tools/pipeline. Some of these things have been around for ages, and if you look at the data system (*.esp files) for Bethesda games, there are useless records in there that go all the way back to Morrowind. Hell, the top record in Skyrim & Fallout 4 is TES4.

I think the and comes in when players assume a fix or update is easy and developers are just lazy. I worked on Halo 5 and there were a few chunks of code from ‘88, pre-Marathon days. Not sure where that got migrated in from.

I never understood why making fun of an accent is racist. Bigoted, likely; stupid, certainly. Japan isn’t homogeneous (though nearly is, thanks to their racist immigration policy), and the mocking of the Japanese accent would apply just as much to a white/black/mixed Japanese citizen as an ethnically Japanese one.
