Anything that gets less then 20mpg, there are enough solid ways to improve efficiency that there is no excuse for things getting anything less.

My first playthrough we tried the middle difficulty got wrecked. Then we switched to easy and the massive boss spawned every level and murked us so we thought even easy was hard.
