I don't think it's the lack of realism that bugs people, more that the plot was kind of a mess and the villain was lame. I actually liked it quite a bit, but did find the plot muddled.

Scrubs in its early seasons was amazing at capturing exactly what it *feels* like to be learning how to practice medicine. Grey's Anatomy tried for that same thing too, and succeeded in maybe the first three episodes before going off the rails.
