For me the most heartbreaking moment is the shot of 20 minutes' worth of flatline ticker tape covering the floor of the trauma room while Mark, covered in sweat, refuses to face the inevitable and keeps attempting resuscitation while the rest of the crew just looks on. Heartbreaking.
