Because they don’t know how much you will end up deducting from your taxes for medical, charity, home mortgage interest, etc. And the taxpayer doesn’t always know what their outside income might be from taxes, rental income, investments.

Considering the vast majority of people will have the new insurance and very few people would pay for separate, private insurance, most hospitals/doctors would fall into line with the new system. Do you know how we can tell? It happened in Canada! And elsewhere!
