Personally I’m going with the “No Body No Death” rule for Andrew. They very conspicuously didn’t show us his face or anything else identifiable. The other Hydra goon was also not shown fleeing with Werner. Therefore, it’s very possible that something went wrong at the hit, and the body was the Hydra goon.

I would say it's a birth defect. The cornea is clear and appears unscarred, and any damage severe enough to destroy the lens and iris would have most likely resulted in loss of the entire eye. He probably survived on his acute sense of hearing and a lot of luck.

the lack of anything resembling a functional iris, or even remnants of an iris probably means that Zeus was born that way. Same goes for the lack of a lens. It requires some pretty serious genetic defects to stop the formation of both, it's a wonder it grew to be a big as it is to be honest.
