That is certain the intent, but the way Joss leaves it at the end showing her flashing back to the ward after she was “cured” and the psychiatrist saying “she’s gone.” It just seemed like he wanted you to doubt which is the real world.

Having a good comedy special is tough! Aziz has only had 1 MAYBE 2 overall really great specials. John Mulaney had one of my favourite specials ever with New in Town, but his other specials are lackluster.
