I liked that the previously on for this episode was pretty much "HEY GUYS, THE MID-EPISODE TWIST IS THAT THE LEVIATHANS ARE DOING THIS." It was very subtle.

Why doesn't the Talking Dead air right after the Walking Dead? It seems like people would be much more likely to leave it on if there wasn't a random Walking Dead rerun on before it.

During Dean's time being tortured in the pit he managed to start the apocalypse. That's probably not a time in Dean's life that really helps him get over his guilt.

It seemed like half this episode was people standing in a basement talking about how to operate the magic flesh pillar and complaining about rich people.
This may have been the least elegant way a show has ever dropped a story arc.
Were people really expecting closure on the multi-season arc of the show. The show doesn't really have huge character arcs and they've pretty much resolved all the will-they or won't-they's anyway, so besides missing out on some weddings, it doesn't seem like the characters need that much closure.
