I think you're talking about a "clinical diagnosis," or, the diagnosis made based on symptoms and signs alone. Endometriosis is not diagnosed on signs + symptoms alone. The disorder results in lesions on ectopic sites, and the presence of these lesions are what confirms the diagnosis.
