The cover "flew" and slided about 20 feet further.
It happened to me this winter in my X-Terra. I was lucky enough not to roll but having your truck's rear lift up 3-5 feet in the air when your totally unsuspecting it is quite mind boggling. Pics of the said manhole in reply...

I work in tires and this a very familiar type of damage. It's usually caused by driving on a flat tire or a blow out caused by underinflation which cause the tire to overheat. It's very common on low profile tires since it's hard to notice a rear flat until it's too late to save the tire.
Low pressure blow out or puncture and driving on it for few miles. Basically the sidewall overheated and broke apart.
For all I know she could have just gotten a flat tire...
