Uh, there was no screaming like a banshee in this case. Read the NYT article. There are other ways that caretakers can gauge consent, and there is nothing to suggest that she didn’t enjoy it in this case. No struggle, etc. They had a loving relationship. Sounds like your example, not so much.

All of the commentators are in arms about this because it appears they’ve decided to completely believe the prosecution’s version of events. That’s usually a bad idea!
