I believe this was the episode that REALLY stirred up some trouble for the series, PR wise. The "Die, Doctor, Die!" cliffhanger was a little more violent than anything that had come before and I believe it's what set off Mary Whitehouse (I think that was her name— could be wrong.)

My alternative explanation could certainly be wrong— but I'm just not sure that at this point in time (that is 1923), pointing out that a woman looks thin/ underweight/ malnourished is necessarily the compliment many would take it to be in 2012.
