Thanks to PBS, a lot of us Americans are very familiar with Doctors 3 through 7, though I stopped watching with five (four, actually, but that was after seeing a lot of five).

It was just that response—that I did not want Tony to be dead, that I thought the show was setting us up for a seventh season we'd never see—that led me away from the conclusion that Tony was dead. In retrospect, it is pretty clear.

Yes, "To save Us All from Satan's Power" established that Pussy had been a rat since Jackie was running things. The DiMeo/Soprano family was lousy with rats. That Jimmy was a rat doesn't mean Pussy wasn't, even at that time.
Note the gesture Tony makes as Gotti "rang that bell." It's a classic jerk-off move and the whole point of the story, so Tony could make that gesture to those assembled jerk-offs (jerks-off?). The story was Tony's subtle way of telling those guys he knew what they were doing and that they could kiss his ass.
