My take is that the terms of her sentence are that she must help the doctor… She mentions a few times in the earlier episodes that "this might pay off the debt". I also think that her prison is what causes her to move backwards in time. She kills the doctor and is put in a prison that is going backwards in time…

BTW, the original intent (no doubt from Verity Lambert's involvment) was to alternate between a science-fiction story and an historical story.
