I second your interpretation. They're showing that it happens a bunch of times between the first time (that we see at least) that Mulder catches on or believes Pam and when they get to the iteration that works.

It wasn't just in Canada. I definitely had it in Ohio and New York. Not a McDonalds fan, and I found it to be one of my least bad options if family went.
