One thing that didn't add up for me was the killing in the cold open. If we're to believe the kids were killing new vamps, why was that one attacking the car and ripping the girl out of it?

From Express, I've got dress shirts, trousers, ties and sweaters. I've never had a quality problem with any of them. My oldest shirt is three years old; it still looks and fits great and doesn't appear to be coming apart at the seems.
I don't know your body type, but as a super thin, tall and lanky guy, I wear Express dress shirts almost exclusively because they sell uncommon sizes on their online store and they fit like a dream. Also, I find the quality pretty great and they use good fabrics.

One thing that irritated me about the episode was in the climactic scene. We get Cas about to kill Dean with his "I need you" line; cut to Naomi: "You have to choose, Castiel, us or them".
Also in Season One, John Winchester was possessed by ole Yellow-Eyes and took a shot to the leg with the Colt, and it didn't kill him.
