Well, in this case, a negative run-in with the show's co-creator and principle producer probably sped the progression along precipitously.

I think there's still room for ambiguity as to whether the Janitor is talking to J.D.'s patients. After all, you see him talking to the patients, but you never really get acknowledgement from the patients that "Yeah, the Janitor told me to get surgery," meaning that it might just be all in J.D.'s paranoid head.

I was going to mention in my review up there that we never do meet J.D.'s mother (though we get ample reference to her still being alive and kicking offscreen) or Turk's father.

It's tough for TV shows to screw up the parental introduction. Even a mediocre TV show can usually pull of some decent casting of a parent or pair of parents for the lead in question, and the writers have probably put a little time and effort already into thinking about why these characters are the way they are.
