As to why Mal didn't expel the egg and check what was inside… @Kumagoro:disqus , your guess is as good as mine.  She clearly could have gotten it removed if she really wanted to.  Perhaps the idea of conducting a medical procedure on herself repulsed her, or her health insurance wouldn't cover it.
