I also don't recall the producers coming out and promising this.  They're making the show they want to make, and you are free to like it or not.  They seem pretty satisfied with the feedback they're getting.

To add to what beema said, it was explicitly shown in the series that she didn't understand that raping and killing on this side of the world would occur.  She got mad at Drogo and he said they had to do it to get enough money to go west, basically.

Interesting that Sansa was able to talk Joffrey out of killing that drunk knight, and The Hound backed her up on it. 

It's different every time.  It shows the cities that will be shown in the episode.  One or two times they showed The Eyrie, and at least once they showed The Twins.  If the Dothraki or The Wall are not shown in the episode, that location is gone from the intro.
