Ethan told Cosima the illness was an unintended side effect of making them infertile, prior to that Cosima assumed their was something wrong with them all. The show hasn't outright said Sarah and Helena are immune until now but the implication was always there.

Sarah and Helena both were born outside of Dyad's control. Amelia must have ran before Dyad could have done the process to make them infertile like the rest, making them immune to the sickness. And they and Kira apparently have a level of healing factor too.
