I appreciate that the show had two character explicitly ask if they're inside David's head or not. That's usually shorthand that they're in the real world.

I think the funniest outcome is that Prometheus is actually an Alternate Oliver.

Yeah, the taxi driver was immune to the poison. Easiest way to tell is that in the first few murders, he has more and more pills in each bottle.

The show has established before that Mycroft thought he was significantly smarter than Moriarty

I just assumed all that was done when she had run of the prison again and could control everything that was going on.

This is a good reading that I think is correct, but it would've been nice if the episode went out of its way to explain any of that.
