Actually, MSG HD was just added to Verizon today.

I think the one thing that hockey fans overlook as a reason for its unpopularity is the 2 intermissions.  That's 35 minutes of dead time in the middle of the game - it's a lot to overcome.  It's a shame that because of the deteriorating playing surface it can't be played in 4 quarters.

I think the NHL has a hard time selling fighting and brutality because it's traditionally a poor television sport and it needs people to go to games to succeed.  The perception is that people and/or corporations willing to pay $120 a ticket aren't really the sorts who can outwardly enjoy gladiator-style entertainment.
