Ramsay was the second "Reek," the version in A Clash of Kings. He took on the original Reek's persona for protection. Once Theon allows Reek to leave Winterfell to get help, he leaves the persona behind. Theon becomes the third "Reek" after Ramsay sacks Winterfell at the end of ACoK.
