I'm on the fence about Wise Blood. Brad Dourif delivers a fine performance, but I'm not sure about the tone of the movie: it was much lighter than I perceived the novel to be. (The score, in particular, is a bit too "screwball comedy"-esque for me.)
I saw that when they first broadcast it in the states. I liked it at the time; I'm wondering if it still holds up. Agreed on the casting and design — though I'm a bit mystified as to how the castle is supposed to flood when it appears to be placed on a big ol' plateau…
"Champ" is a favorite of mine.
The AV Club needs an inventory of incompetent TV/film doctors; Dr Clarkson certainly belongs on such a list. Also, as mentioned, Spaceman. And Zoidberg. And…
I don't dislike Lavinia, but nor did I ever like her. She never seemed to have any character traits besides "selfless".
That does seem to be the most plausible option, at this point. If somebody else had done it, there would've been enough foreshadowing that it would be no surprise — like so many other plot points…

There is at least one little jab at Cora's origins, in season one: Mary tells her something to the effect of "you're American; you wouldn't understand".
I have no relevant experience to speak of, but this was exactly what I had assumed.
To be fair, good news is generally less interesting.
