I think that the doll was placed in Hoyt and Jessica's house by Summer, that crazy girl Hoyt's mom foisted on him last season. She was obsessed with dolls too (didn't she even bring some to Merlotte's once on a date??). Then, after he broke up with her, there was a shot of the doll under his bed, I believe.

I feel like I recognized the voice, but I couldn't place it. But the opening recap announcer sounded a lot like Demitri Martin to me.
