that's called the"Bus" effect based first used in the original Cat People. A tense scene and suddenly a bug bus drives by loudly and everyone messes their pants.
aside from the Mexican drug lord part this is an exact carbon copy of the original… right down to the crime and side characters. I imagine the show will play out like the original which is actually a good thing. It goes some places you won't expect.

a few comments. The doc is slowly realizing she mis-diagnosed Dexter as a tot. He shouldn't have feels or concern for his sister.
