I haven't seen this but it sounds a helluvalot like Joe the King. Which was directed by Frank Whaley, best known for playing Brett in Pulp Fiction.
At first, I found the bad behaviour in this episode a little jarring. Like when the mother makes that comment about Adam being odd. etc. That line seemed extreme and not consistent with the caring loving mother she seemed to personify up to this point.

The writing is so nuanced. I love how that line emphasizes her lack of medical knowledge - and therefore her dissimilarity with her cousin.

I love this movie.
The was actually pretty funny. I laughed at John Madden.
That's why the Louis episode was so fantastic. I love how it oscillated between total greatness (the monologue and Lincoln) and joyous stupidity (the make-out). Some sketches combined both qualities (the horn blower guy). It felt like any moment it could all come crashing down.
