As you can probably surmise, the piercing goes through the clitoral hood - the clitoris itself is not pierced. Usually it is done with a curved barbell which sits against the clitoris, so playing with the jewelry is arousing and the result is akin to increasing the size of or exposing the clitoris.

I've had at least six oral surgeries so I can kind of understand, but I did my research and made sure that I had my VCH done by a highly recommended piercer who had extensive experience with female genital piercings.
