That is when people speed ball it or shoot it directly. In controlled doses, it isn't quite as bad. It's the same as heroin. Shooting that shit into your veins will fuck you up, but in pill form at low dosages, it does it's job. That isn't to say it is "safe," but it isn't as bad.
