yes. it definitely deserves a spot. It was sort of an inspiration for movies like scream. I thought it was ahead of it's time and unlike most of the other slashers from the 80's.

Actually... I encountered some enemies, in that empty world, that I didn't kill during the first playthrough. Scared the shit outta me every time. Running around a corner and this bad guy jumping at me.

There even was a 'official' sequel called 'Heart of the Alien'. The original developer (chahi) wasn't involved in the game but gave his permission to make the sequel. It was only released for the Sega.
