You may not be far off with the 3-D printing thing. Here in Philly Penn Vet 3-D printed an implant to replace part of a dog's skull. The dog had a large tumor and the entire top of his skull had to be removed. You can't tell at all - his head looks completely normal.

And straight sizes are scaled from the midpoint, i.e., Size 8. Adding the larger sizes definitely would pose a challenge. As you said, with such small margins, most manufacturers don't want to be the first to try.
