Sometimes it just happens. There could have been a hairline fracture that went unnoticed by QA. Also, you don't need to preheat your stone. Were you preheating your stone and then putting frozen foods onto it?
Having it dark like that is actually better. All the oils and grease that are absorbed into the stone help make it non stick. When you clean it, don't use soap and water. Scrape the burned off food and use just straight water if you need to use water. The darker the stone gets, the better it is! :)
Also, be careful not to get water on the hot stone. That can cause it to crack, too. When you take it out of the oven, make sure whatever surface you're putting it on is dry.

I have a lot of baking stones. I've got a pizza stone, I've got two flat stones (cookies!), one small one, a muffin pan, a small ridged one that does bacon awesome in the microwave (or the oven, but it's quicker in the microwave), a couple of casserole dishes, and a 4x mini loaf pan. I'm a total stoner.
If you're cooking frozen stuff, don't preheat the stone. You will have to adjust your cooking time, but it's totally fine to cook frozen food on a (non preheated) stone.
That's actually a good thing. Cooking high fat or greasy things on it will help it become non stick. The darker your stone is, the better. It's kind of like seasoning a cast iron pan.
