I have a 2015 JGC and can see how this occurred. The shifter is easy to use but if you are not paying attention, you can most certainly put the car in reverse or neutral and not realize it.

I'm assuming that's because the Audi engages the electronic parking brake if you exit the vehicle without putting in park, just like the BMW shifter of the same/similar design.
