There is. A DOT medical card is only a requirement to get a CDL learner’s permit, which is needed to begin training for a CDL. 

I completed CDL school (class A; 18 wheeler) only seven weeks ago and have been working in the industry for just two weeks. By far the biggest focus both in school and with my new employer is safety; for me the driver and other motorists. I now routinely haul a combined weight of 80,000 lbs in the far western suburbs…

...but that’s not important right now.
