If you're looking for the unforgiving aspect, try getting online. That alone should do it for you. In all seriousness, free roam is filled with 15 hostiles that will drive as fast as possible to your spawn to try their best to kill you over and over again while your medical bills pile up and all that money you grinded…

I dont really kill civilians in GTA games (I'm a decent enough driver) so my characters are only really killing like drug dealers and cops, which are pretty much just as bad in the GTA universe
