It was after a timeskip in DA2. So Hawke revived Flemeth sometime (can't remember if it was a year or 2) after the events of DA:O despite Hawke receiving the item to revive her before the events of DA:O.

Its okay though. With ME3 they made most of those decisions not matter in the long run anyway as things like the Rachni Queen decision were rendered moot and really only had an impact on a random set of numbers in that galactic readiness score.

DA2 sorta established that even if you did really kill her, she had sent the magic equivalent of a save file with the Hawkes and instructions to take it to a respawn point for her.

If you played on Xbox you'd hafta use the Genesis dlc but even that automatically chooses some of the minor decisions. If you played on PC, there are programs where you can make a save that has all the decisions you want flagged.
